Section 6500.21. Reporting of deaths

(a) A death report shall be completed on a form specified by the Department and sent to the county mental retardation program of the county in which the home is located, the funding agency and the regional office of mental retardation, within 24 hours after a death of an individual occurs.
(b) An investigation shall be initiated and oral notification of the county mental retardation program of the county in which the facility is located, the funding agency and the appropriate regional office of mental retardation shall be given within 24 hours after an unusual or unexpected death occurs.
(c) A copy of death reports shall be kept.
(d) The individuals family or guardian shall be immediately notified of the death of an individual.
