Section 6210.44. Evaluations

(a) Before admission to a facility, or before authorization for payment, an interdisciplinary team of health professionals shall make a comprehensive medical, social and psychological evaluation of each applicants or recipients need for intermediate care for the mentally retarded. The psychological evaluation shall be completed within 3 months prior to admission.
(b) If a recipient moves from one facility to another facility, this is not considered a new admission and new evaluations as required in subsection (a) are not required, if the prior evaluations are transferred with the recipient.
(c) Medical, social and psychological evaluations shall be recorded in the recipients medical record and if applicable on forms specified by the Department.
