Section 45.15. Duplicate certificates

Duplicate license certificates and wallet cards will be issued only upon submission by the licensee of a notarized statement specifying the reason for the requestFees as prescribed by the Bureau of Professional and Occupational Affairs Fee Act (63 P. S. § § 1401-101—1401-501) shall be charged for duplicate wall certificates.
