Section 13.66. Refusal of preceptor to furnish affidavit

(a) A preceptor who refuses to furnish a resident intern with the affidavit of completion of full or part-time service shall immediately submit to the Board a statement as to the reason for the refusal. If the information is not furnished or if there appears to be no proper basis for the refusal, the Board may, after due notice and hearing, take disciplinary action against the license of the preceptor.
(b) If a preceptor refuses to furnish the required affidavit, an applicant may submit with the application a statement that the preceptor refused to honor the applicants request for an affidavit, and an affidavit from another licensed funeral director who has personal knowledge of the applicants resident intern training containing the same information that is required in connection with an affidavit of a preceptor.
