 On June 15, 2016, Susan Crocetto, LPN, license No. PN085350L, last known of Clifton Heights, Delaware County, was indefinitely suspended retroactive to July 22, 2015, based upon her failing to enter a voluntary recovery program and failing to attend her mental and physical examination.

 Individuals may obtain a copy of the adjudication by writing to Board Counsel, State Board of Nursing, P.O. Box 69523, Harrisburg, PA 17106-9523.

 This order represents the final State Board of Nursing (Board) decision in this matter. It may be appealed to the Commonwealth Court of Pennsylvania by the filing of a petition for review with that court in accordance with the Pennsylvania Rules of Appellate Procedure. Individuals who take an appeal to the Commonwealth Court must serve the Board with a copy of their petition for review.
