    APPLICATIONS RECEIVED UNDER THE PENNSYLVANIA CLEAN STREAMS LAW AND THE FEDERAL CLEAN WATER ACT

    [National Pollution Discharge Elimination System Program (NPDES)]

    DISCHARGE OF CONTROLLED INDUSTRIAL WASTE AND SEWERAGE WASTEWATER

    (Part I Permits)

       The following parties have applied for an NPDES permit to discharge controlled wastewaters into the surface waters of this Commonwealth. Unless otherwise indicated on the basis of preliminary review and application of lawful standards and regulations, the Department of Environmental Protection (Department) proposes to issue a permit to discharge, subject to certain effluent limitations and special conditions. These proposed determinations are tentative.

       Where indicated, the EPA, Region III, Administrator has waived the right to review or object to this proposed permit action under the waiver provision 40 CFR 123.6E.

       Persons wishing to comment on the proposed permit are invited to submit a statement to the office noted above the application within 30 days from the date of this public notice. Comments received within this 30-day comment period will be considered in the formulation of the final determinations regarding this application. Responses should include the name, address and telephone number of the writer and a concise statement to inform the Department of the exact basis of a comment and the relevant facts upon which it is based. A public hearing may be held if the responsible office considers the public response significant.

       Following the 30-day comment period, the Water Management Program Manager will make a final determination regarding the proposed permit. Notice of this determination will be published in the Pennsylvania Bulletin at which time this determination may be appealed to the Environmental Hearing Board.

       The application and related documents, proposed effluent limitations and special conditions, comments received and other information are on file and may be inspected and arrangements made for copying at the office indicated above the application.

For the purpose of evaluating effluent requirements, the existing downstream potable water supply intake considered during the evaluation was the Pottstown Borough Authority intake located on the Schuylkill River in Montgomery County. The discharge is not expected to impact any potable water supply.

       The following effluent limitations are proposed for discharge to the receiving waters, known as the Youghiogheny River, which are classified as a warm water fishery with existing and/or potential uses for aquatic life, water supply and recreation. The first downstream potable water supply intake from this facility is the Westmoreland County Municipal Authority, McKeesport Plant, on the Youghiogheny River.

       Outfall 001:  Existing average design flow of 0.40 mgd, effective until the sewage treatment plant expansion is completed and operational.

    Proposed NPDES Permit Renewal Actions for Minor Sewage Discharges

       The following parties have applied to renew their current NPDES permits to allow the continued discharge of controlled wastewaters into the surface waters of this Commonwealth. The Department of Environmental Protection (Department) has made a tentative determination to renew these permits and proposes to issue them, subject to their current permit effluent limitations, and monitoring and reporting requirements, with appropriate and necessary updated requirements to reflect new or changed regulations and other requirements. The updates may include, but will not be limited to, applicable permit conditions and/or requirements addressing combined sewer overflows (CSOs), municipal sewage sludge management and total residual chlorine control (TRC). Major changes to or deviations from the terms of the existing permit will be documented and published with the final Department actions.

       The EPA, Region III, Regional Administrator has waived the right to review or object to these proposed permit actions under the waiver provision 40 CFR 123.6E.

       Persons wishing to comment on the proposed permits are invited to submit a statement to the Field Office indicated as the office responsible, within 30 days from the date of this public notice. Comments received within this 30-day period will be considered in the formulation of the final determinations regarding these applications and proposed permit actions. Comments should include the name, address and telephone number of the writer and a brief statement to inform the Field Office of the basis of the comment and the relevant facts upon which it is based. A public hearing may be held if the Field Office considers the public response significant.

       Following the 30-day comment period, the Water Management Program Manager will make a final determination regarding the proposed permit action. Notice of this determination will be published in the Pennsylvania Bulletin at which time this determination may be appealed to the Environmental Hearing Board.

       A.  1799503.  The Department has received a construction permit application from Irvona Municipal Authority Water Treatment Plant, Berwind Street, P. O. Box 247, Irvona, PA 16656, Irvona Borough, Clearfield County, to construct a water treatment plant to be located at Well No. 2. The plant is to treat water from the Hockenberry Run Reservoir and Well No. 1 and 2.

       A.  Minor Amendment.  The Department has received a permit application from the Mahoning Township Municipal Authority, 1101 Bloom Road, Danville, PA 17821. The permit is for construction of an underground booster pump station at the southwest corner of the intersection of Red Lane and Kaseville Road. Construction will include the installation of two hydroconstant pumps, each at 40 gpm, supply and discharge piping and electrical service and controls. This pump station is to supply boosted water supply pressure to a home in the Edgewood Subdivision located northeast of the intersection of Red Land and Kaseville Road.

       A.  1499505.  The Department has received a permit application from Eagle Creek Partnership, 1901 East College Avenue, State College, PA 16801, Union Township, Centre County. This project consists of developing a recently drilled water well (well no. EC-1) as a permanent water source for the Eagle Creek Mobile Home Park. Additionally, water treatment will be accomplished by filtration to reduce manganese and total solids to below the required maximum permissible levels with chlorine disinfection.

    LAND RECYCLING AND ENVIRONMENTAL REMEDIATION

    Preamble 1


    Acknowledgment of Notices of Intent to Remediate submitted under the Land Recycling and Environmental Remediation Standards Act (35 P. S. §§ 6026.101--6026.908).

       Sections 302, 303, 304 and 305 of the Land Recycling and Environmental Remediation Standards Act (act) require the Department of Environmental Protection (Department) to publish in the Pennsylvania Bulletin an acknowledgment noting receipt of any Notices of Intent to Remediate. An acknowledgment of the receipt of a Notice of Intent to Remediate is used to identify a site where a person proposes to, or has been required to, respond to a release of a regulated substance at a site. Persons intending to use the background, Statewide health standard, the site-specific standard, or who intend to remediate a site as a special industrial area, must file a Notice of Intent to Remediate with the Department. A Notice of Intent to Remediate filed with the Department provides a brief description of the location of the site, a list of known suspected contaminants at the site, the proposed remediation measures for the site and a description of the intended future use of the site. A person who demonstrates attainment of one, or a combination of the cleanup standards, or who receives approval of a special industrial area remediation identified under the act, will be relieved of further liability for the remediation of the site for any contamination identified in reports submitted to and approved by the Department. Furthermore, the person shall not be subject to citizen suits or other contribution actions brought by responsible persons not participating in the remediation.

       Under sections 304(n)(1)(ii) and 305(c)(2) of the act, there is a 30-day public and municipal comment period for sites proposed for remediation using a site-specific standard, in whole or in part, and for sites remediated as a special industrial area. This period begins when a summary of the Notice of Intent to Remediate is published in a newspaper of general circulation in the area of the site. For the sites identified as proposed for remediation to a site-specific standard or as a special industrial area, the municipality, within which the site is located, may request to be involved in the development of the remediation and reuse plans for the site if the request is made within 30 days of the date specified. During this comment period the municipality may request that the person identified, as the remediator of a site, develop and implement a public involvement plan. Requests to be involved, and comments, should be directed to the remediator of a site.

       For further information concerning the content of a Notice of Intent to Remediate, contact the Environmental Cleanup Program Manager in the Department's Regional Office under which the notice appears.        The Department of Environmental Protection (Department) has developed an integrated plan approval, State operating permit and Title V operating permit program. This integrated approach is designed to make the permitting process more efficient for the Department, the regulated community and the public. This approach allows the owner or operator of a facility to complete and submit all the permitting documents relevant to its application one time, affords an opportunity for public input and provides for sequential issuance of the necessary permits.

       The Department has received applications for plan approvals and/or operating permits from the following facilities. Although the sources covered by these applications may be located at a major facility, the sources being installed or modified do not trigger major new source review or prevention of significant deterioration requirements.

       Copies of these applications, subsequently prepared draft permits, review summaries and other support materials are available for review in the Regional Offices identified in this notice. Persons interested in reviewing the application files should contact the appropriate regional office to schedule an appointment.

       Persons wishing to file protests or comments on the proposed plan approval and/or operating permits must submit the protest or comment within 30 days from the date of this notice. Interested persons may also request that a hearing be held concerning the proposed plan approval and operating permit. Comments or protests filed with the Department's Regional Offices must include a concise statement of the objections to the issuance of the plan approval or operating permit and relevant facts which serve as the basis for the objections.     Notice of Intent to Issue Title V Operating Permits

       Under 25 Pa. Code § 127.521, notice is given that the Department of Environmental Protection (Department) intends to issue a Title V Operating Permit to the following facilities. These facilities are major facilities subject to the operating permit requirements under Title V of the Federal Clean Air Act and 25 Pa. Code Chapter 127, Subchapters F and G (relating to operating permit requirements; and Title V operating permits).

       The following permit applications to conduct mining activities have been received by the Department of Environmental Protection (Department). A copy of the application is available for inspection at the District mining office indicated above each application. Where a 401 water quality certification is needed for any aspect of a particular proposed mining activity, the submittal of the permit application will serve as the request for the certification.

       Written comments or objections, or requests for informal conferences on applications, may be submitted by any person or any officer or head of any Federal, State or local government agency or authority to the Department at the same address within 30 days of this publication, or within 30 days after the last publication of the applicant's newspaper advertisement, as provided by 25 Pa. Code §§ 77.121--77.123 and 86.31--86.34 (relating to public notices of filing of permit applications, opportunity for comment, and informal conferences).

       Where any of the mining activities listed below will have discharges of wastewater to streams, the Department will incorporate NPDES permits into the mining activity permits issued in response to these applications. The NPDES permits will contain, at a minimum, technology-based effluent limitations (as described in the Department's regulations--25 Pa. Code §§ 77.522, 87.102, 88.92, 88.187, 88.242, 89.52 and 90.102) for iron, manganese, suspended solids, settleable solids, alkalinity and pH. In addition to the above, more restrictive effluent limitations, restrictions on discharge volume, or restrictions on the extent of mining which may occur will be incorporated into a mining activity permit when necessary for compliance with water quality standards (in accordance with 25 Pa. Code Chapters 93 and 95). Persons or agencies which have requested review of the NPDES permit requirements for a particular mining activity within the above-mentioned public comment period will be provided with a 30-day period to review and submit comments on those requirements.

       Written comments or objections should contain the name, address and telephone number of persons submitting comments or objections; application number; and a statement of sufficient detail to inform the Department on the basis of comment or objection and relevant facts upon which it is based. Requests for an informal conference must contain the name, address and telephone number of requestor; application number; a brief summary of the issues to be raised by the requestor at the conference; and a statement whether the requestor desires to have the conference conducted in the locality of the proposed mining activities.

       E38-127.  Encroachment. Whitmoyer Laboratories, 99 Creamery Rd., Myerstown, PA 17067. To perform remedial excavation to bedrock for removal of onsite contaminated soils/sediments, backfilling of all excavated areas, and placement of a clean soil cover over onsite areas located within the floodway of the Tulpehocken Creek (TSF) and the Union Canal. The sewer line begins at the western end of the Borough of Emporium (Emporium, PA Quadrangle N:  1.7 inches; W:  16.5 inches) and extends west along Route 120 to a location approximately 3 miles from the Borough (West Creek, PA Quadrangle N:  21.1 inches; W:  6.4 inches) in the Borough of Emporium and Shippen Township, Cameron County. Total estimated stream disturbance is 300 feet and total wetland impact is 0.53 acre.

       E30-190.  Encroachment. PA Department of Transportation, Engineering District 12-0, P. O. Box 459, Uniontown, PA 15401. To remove the existing bridge and to construct and maintain a precast concrete spread box beam bridge having a single normal span of 80.0 feet and an underclearance of 13.2 feet across South Fork Tenmile Creek (HQ-WWF). Also to conduct channel cleaning within 50 feet upstream and 50 feet downstream from the bridge. The project is located along S. R. 0018 approximately 200 feet south of its intersection with S. R. 0021 (Holbrook, PA Quadrangle N:  22.5 inches; W:  4.5 inches) in Center Township, Greene County.

       E65-734.  Encroachment. Ligonier Township Supervisors, 18 Old Lincoln Highway West, Ligonier, PA 15658-8763. To construct and maintain earthen dikes having a maximum height of 1.5 feet and extending approximately 700 l. f. along an unnamed tributary to Mill Creek (CWF) and to remove sediment deposits and debris from the stream banks, for the purpose of minimizing flooding.        The following parties have submitted (1)  Notices of Intent (NOIs) for coverage under General NPDES Permit(s) to discharge wastewater into the surface waters of this Commonwealth; (2)  NOIs for coverage under General Permit(s) for Beneficial Use of Sewage Sludge or Residential Septage by Land Application in Pennsylvania; or (3)  Notifications for First Land Application of Sewage Sludge.

       The approval of coverage under these General Permits may be subject to one or more of the following:  pollutant or effluent discharge limitations, monitoring and reporting, pathogen and vector attraction reduction requirements, operational standards, general requirements, management practices and other conditions set forth in the respective general permit. The Department of Environmental Protection (Department) has reviewed the NOIs and determined that they comply with administrative requirements of the respective permit application. Also, the Department has evaluated the First Land Application of Sewage Sludge for the sites applying for coverage under PAG-7, PAG-8 and PAG-9 and determined that the sites are suitable for land application of sewage sludge.

       The EPA Region III Regional Administrator has waived the right to review or object to this permit action under the waiver provision:  40 CFR 123.24.

    HAZARDOUS SITES CLEANUP

    Under the Act of October 18, 1988

    Notice of De Minimis Settlement under the Hazardous Sites Cleanup Act

    Mangan Property Site
    Moore Township, Northampton County

       The Department of Environmental Protection (Department), under the authority of the Hazardous Sites Cleanup Act (35 P. S. §§ 6020.101--6020.1305) (HSCA), has entered into a consent order and agreement for de minimis settlement with Keith Tamborelli (Tamborelli). The settlement is in regards to the Department's costs incurred at the Mangan Property Site (site). The site is located off East Beersville Road in Moore Township, Northampton County, PA.

       The site formerly consisted, in part, of a storage shed attached to a barn where hazardous substances, including pesticides and herbicides, were stored. The structure, approximately 10 feet by 15 feet in size, contained residue from the storage of these materials. The wood flooring and side walls of the shed were contaminated with hazardous substances prior to the purchase of the property by Tamborelli. Tamborelli purchased the parcel of property which contained the storage shed, on August 20, 1998.

       The site also consisted of two parcels of property, owned by Patrick J. Mangan (Mangan), which are located off of East Beersville Road. On September 3 and 4, 1998, the Department conducted a response action at the portion of the site which was owned by Mangan. During the response the Department and its contractor identified the areas on the two parcels owned by Mangan where hazardous substances had been dumped, collected samples for analysis and excavated and removed from the site the wastes and contaminated soils. The Department removed for proper disposal five rolloff containers of contaminated materials and one 55 gallon drum containing smaller containers of chemicals. The Department began the removal and subsequent disposal of additional contaminated soil and materials on October 26, 1998, and completed this second phase of the response on November 2, 1998.

       On September 22, 1998, the Department met with its contractor to plan the removal and safe disposal of the materials contaminated with hazardous substances in and below the storage shed attached to the barn. This work was completed on November 2, 1998.

       During the response action the Department identified the following hazardous substances and contaminants in the soil collected from the site:  fonofos (dyfonate), hexachlorocyclohexane (lindane), toxaphene and alachlor.

       On May 5, 1999, the Department collected soil samples from the area where the shed had been located. Analytical results of those samples demonstrated that soil which had been left on the site had concentrations of site-related contaminants below cleanup standards as promulgated by the Land Recycling and Environmental Remediation Standards Act (35 P. S. §§ 6026.101--6026.908). Under the proposed consent order and agreement the Department will receive a total of $1,000 from Tamborelli.

       This notice is being provided under sections 1113 of HSCA. The proposed consent order and agreement is subject to final approval by the Department following the receipt and review of comments submitted by the public. The proposed consent order and agreement that contains the specific terms of the settlement is available for public review and comment. The proposed consent order and agreement can be examined from 8 a.m. to 4 p.m. at the Department's Bethlehem District Office at 4530 Bath Pike (Route 512) in Hanover Township, Northampton County. A public comment period on the proposed consent order and agreement will extend until November 10, 1999. Persons may submit written comments regarding the proposed consent order and agreement to the Department by sending them to James Kunkle, DEP Project Officer, at the DEP Bethlehem Office at 4530 Bath Pike (Route 512) in Hanover Township, Northampton County or by delivering them to that office in person.

       Provisions of 25 Pa. Code § 250.8 Administration of Land Recycling Program requires the Department of Environmental Protection (Department) to publish in the Pennsylvania Bulletin a notice of its final actions on plans and reports. A final report is submitted to document cleanup of a release of a regulated substance at a site to one of the remediation standards of the Land Recycling and Environmental Remediation Standards Act (act). Plans and reports required by provisions of the act for compliance with selection of remediation to a site-specific standard, in addition to a final report, include a remedial investigation report, risk assessment report and cleanup plan. A remedial investigation report includes conclusions from the site investigation, concentration of regulated substances in environmental media, benefits of reuse of the property, and in some circumstances, a fate and transport analysis. If required, a risk assessment report describes potential adverse effects caused by the presence of regulated substances. A cleanup plan evaluates the abilities of potential remedies to achieve remedy requirements. Tarro, Environmental Manager, Pennsylvania Air National Guard, 62 Olmstead Boulevard, Middletown, PA 17057, has submitted a Final Report concerning the remediation of site soil contaminated with BTEX, PHCs and PAHs. The Final Report demonstrated attainment of the Statewide health standard and was approved by the Department on July 8, 1999.

       Lock Haven University of Pennsylvania--Former KCSD Vocational Agriculture Center and Athletic Fields, City of Lock Haven, Clinton County. Converse Consultants on behalf of their client Lock Haven University of Pennsylvania, has submitted a Final Report concerning the remediation of soil contaminated with heavy metals. The Final Report demonstrated attainment of the background standard and was approved by the Department on July 7, 1999.

       Stroehmann Bakeries Roll Plant, Old Lycoming Township, Lycoming County. Converse Consultants on behalf of their client Stroehmann Bakeries, P. O. Box 976, Horsham, PA 19044, has submitted a Final Report concerning the remediation of site soil and groundwater contaminated with solvents, BTEX and PHCs.

       The Department of Environmental Protection (Department) has taken the following actions on previously received Dam Safety and Encroachment permit applications, requests for Environmental Assessment approval and requests for Water Quality Certification under section 401 of the Federal Water Pollution Control Act (33 U.S.C.A. § 1341(a)).

       Persons aggrieved by this action may appeal, under section 4 of the Environmental Hearing Board Act (35 P. S. § 7514) and 2 Pa.C.S. §§ 501--508 and 701--704 (relating to the Administrative Agency Law) to the Environmental Hearing Board, 400 Market Street, Floor 2, P. O. To remove accumulated peat and organic material from a 30-foot wide swath around the perimeter of K Lake Reservoir at Pocono Country Place residential community. The project involves partial drawdown of the lake to allow restoration of original lakebed elevations by exposing and removing peat and other organic material. The project involves 4.6 acres of disturbance and is located on the east side of S. R. 0196, along Lakeside Drive in Pocono Country Place, Section K (Buck Hill Falls, PA Quadrangle N:  16.5 inches; W:  14.7 inches) in Coolbaugh Township, Monroe County.

       E35-294. Encroachment. Northeast Real Estate Development, Inc., P. O. Box 128, Waverly, PA 18471. To fill in 0.28 acre of wetlands for the construction of Village Square at Blakely, a 56 residential lot subdivision. The project is located approximately 0.8 mile east of S. R. 1023 and S. R. 0247 (Olyphant, PA Quadrangle N:  20.1 inches; W:  10.5 inches) in Blakely Borough, Lackawanna County. The permittee is required to provide for 0.28 acre of replacement wetlands by participating in the Pennsylvania Wetland Replacement Project.

       E52-160. Encroachment. Charles Prohaska, R. R. 2, Box 470, Dingmans Ferry, PA 18328. To repair and maintain an existing low-flow road crossing of Hornbecks Creek, with work consisting of replacing the deteriorated 6-foot-diameter CMP culvert with either a 6-foot-diameter or a 9.3-foot × 6.3-foot arch CMP culvert. The road serves as a driveway to a single residence. The project is located on the east side of T-314 (known as Log-N-Twig Road), approximately 2,000 feet southwest of the intersection of T-314 and S. R. 2001 (Lake Maskenozha, PA-NJ Quadrangle N:  13.6 inches; W:  6.8 inches), in Delaware Township, Pike County.

The amount of wetland impact is considered a de minimis impact of 0.001 acre and wetland mitigation is not required. This permit also includes 401 Water Quality Certification.

       E06-514. Encroachment. John Henry, 1025 Hunter Road, Mohnton, PA 19540. To construct and maintain an excavated pond impacting 0.46 acre of wetlands adjacent to Wyomissing Creek at a point downstream of Hunter Road (Reading, PA Quadrangle N:  2.5 inches; W:  16.7 inches) in Cumru Township, Berks County. The permittee has met the wetland replacement requirement by participating in the Pennsylvania Wetland Replacement Project. This permit also includes 401 Water Quality Certification.

       E07-302. Encroachment. Zane Johnsonbaugh, 3200 S. 10th Avenue, P. O. Box 2326, Altoona, PA 16603. To place fill in 0.1 acre of wetlands for the purpose of developing a 8.671 acre commercial property to include a warehouse, general office building and maintenance structures for an existing trucking business located approximately 0.3 mile north of the I-99 Pinecroft Interchange (Bellwood, PA Quadrangle N:  12.75 inches; W:  9.5 inches) in Antis Township, Blair County. The permittee is required to provide 0.1 acre of replacement wetlands. This permit also includes 401 Water Quality Certification.

       E50-186. Encroachment. Regina Crotsley, R. R. 1, Blain, PA 17006. To restore an existing damaged bridge by replacing the deck, repairing or replacing damaged abutments and piers and regrading the stream banks across the channel of Bull Run (HQ-CWF) at a point approximately 5,000 feet upstream of Manassa Church (Andersonburg, PA Quadrangle N:  10.25 inches; W:  17.2 inches) in Jackson Township, Perry County. This permit was issued under section 105.13(e) ''Small Projects.'' This permit also includes 401 Water Quality Certification.

       E67-616. Encroachment. John Argento, 400 Hunting Park Lane, York, PA 17402. To fill 0.47 acre of wetlands for the purpose of developing lots 63A, 63B, 63C and 63D within the existing Chapelwood Estates residential subdivision at a point located north of Kreutz Creek southeast of the intersection of Dyan Road and Dietz Road (Red Lion, PA Quadrangle N:  15.0 inches; W:  16.5 inches) in Windsor Township, York County. Permittee agrees to construct 0.47 acre of replacement wetlands. This permit also includes 401 Water Quality Certification.

       E67-625. Encroachment. Donald Smith, Shrewsbury Commercial Tract, 450 Spangler Road, New Oxford, PA 17350. To place fill in 0.72 acre of jurisdictional wetlands to construct a retail shopping center and grocery store on the Shrewsbury Commercial Tract located south of Shrewsbury on New Freedom Road (Glen Rock, PA Quadrangle N:  0.0 inch; W:  8.1 inches) in Shrewsbury Township, York County. The permittee is required to provide a minimum of 0.94 acre of replacement wetlands. This permit also includes 401 Water Quality Certification.

       E67-663. Encroachment. PA Department of Transportation, Engineering District 8-0, 2140 Herr Street, Harrisburg, PA 17103. To remove an existing structure and to construct and maintain a prestressed concrete I-beam bridge having a clear span of 75 feet at a skew angle of 37° and an underclearance of 13.4 feet over Otter Creek (CWF) on SR 2020, Section 001, Segment 0080, Offset 1829, Station 4+51.22 (Gum Tree Road) for the purpose of roadway improvements located 170 feet upstream of its existing location approximately 0.75 mile south of Burkholder Road (Safe Harbor, PA Quadrangle N:  2.8 inches; W:  5.4 inches) in Chanceford Township, York County. This permit also includes 401 Water Quality Certification.

       E14-341. Encroachment. Ferguson Township, 3147 Research Drive, State College, PA 16801. To remove the existing structure and to construct and maintain a precast rigid frame culvert with a 24-foot span, a 4-foot rise and being 24 feet in length with a natural bottom to carry T-307 over Halfmoon Creek, and to regrade approximately 25 feet of channel bank to provide a smooth transition into the proposed inlet, and to construct and maintain a scour hole at the outlet end to the culvert. The project is located on T-307 approximately 0.4 mile east of the intersection of T-307 with Marengo Road (Franklinville, PA Quadrangle N:  21.6 inches; W:  3.7 inches) in Ferguson Township, Centre County.

       E18-270. Encroachment. Clinton County Solid Waste Authority, Wayne Township Landfill, SR 1005, Landfill Lane, P. O. Box 209, McElhattan, PA 17748-0209. To maintain an existing 8-inch diameter HDPE gas pipeline connected to the deck of an existing abandoned railroad bridge over the West Branch Susquehanna River approximately 0.4 mile north of the intersection of SR 2012 with T-425 (Jersey Shore, PA Quadrangle N:  7.3 inches; W:  11.7 inches) in Wayne Township, Clinton County.        E18-280. Encroachment. Michael K. Hanna, 50 Sylvan Drive, Lock Haven, PA 17745. To construct a 6` × 8` × 1-1/2` concrete pad and a 20` × 30` deck/canopy supported by six pillars in the floodway of the West Branch Susquehanna River (Lock Haven, PA Quadrangle North:  6.25 inches; W:  13.25 inches) in Woodward Township, Clinton County. This permit was issued under section 105.13(e) ''Small Projects.'' This permit also includes 401 Water Quality Certification.

       E19-193. Encroachment. Antonio D. Rado and Lucy M. Rado, R. R. 1, Box 3B, Stillwater, PA 17878. To construct, operate and maintain a minor road crossing a contributory channel of Fishing Creek for private, single resident access. The minor road crossing shall be constructed with two corrugated metal culvert pipes. Each culvert pipe shall be depressed 0.5-feet, have a minimum span of 4.1-feet, have a minimum rise of 2.75-feet and have a minimum length of 20-feet that is located along the western right-of-way of SR 0487 approximately 200-feet north of the intersection of SR 1035 and SR 0487 (Benton, PA Quadrangle N:  5.8 inches; W:  0.1 inch) in Stillwater Borough, Columbia County. This permit does not authorize any permanent or temporary impacts to jurisdictional wetlands associated to Fishing Creek. This permit was issued under section 105.13(e) ''Small Projects.''
