 On June 14, 2016, Rachel Keridwen Carlson, last known of Kempton, Berks County, was denied licensure, based upon not having met all of the educational requirements set forth.

 Individuals may obtain a copy of the adjudication by writing to Megan E. Castor, Board Counsel, State Board of Social Workers, Marriage and Family Therapists and Professional Counselors, P.O. Box 69523, Harrisburg, PA 17106-9523.

 This adjudication and order represents the final State Board of Social Workers, Marriage and Family Therapists and Professional Counselors (Board) decision in this matter. It may be appealed to the Commonwealth Court of Pennsylvania by the filing of a petition for review with that court in accordance with the Pennsylvania Rules of Appellate Procedure. Individuals who take an appeal to the Commonwealth Court must serve the Board with a copy of their petition for review. The Board contact for receiving service of the appeals is the previously-named Board counsel.
