In the Matter of the Automatic Suspension of the License to Practice Practical Nursing of Connie Leininger Hafer, L.P.N., a.k.a. Connie Leininger; Doc. No. 0498-51-05

   Connie Leininger Hafer, a.k.a. Connie Leininger of Mohnton, Berks County, was automatically suspended by the State Board of Nursing (Board) based on a guilty plea or nolo contendere to a misdemeanor of the Drug Act.

   Individuals may obtain a copy of the adjudication by writing to Carole L. Clarke, Board Counsel, State Board of Nursing, P. O. Box 2649, Harrisburg, PA 17105-2649.

   This final order and automatic suspension represent the final Board decision in this matter. It may be appealed to the Commonwealth Court of Pennsylvania by the filing of a petition for review with that court in accordance with the Pennsylvania Rules of Appellate Procedure. Individuals who take an appeal to the Commonwealth Court must serve the Board with a copy of the petition for review.
