Nazareth Hospital (Lavin); Prehearing [34 Pa.B. 3083] Appeal of Nazareth Hospital (Lavin) under the Medical Care Availability and Reduction of Error (MCARE) Act (40 P. S. §§ 1303.101--1303.910);

No. MM04-05-040On or before June 23, 2004, the appellant shall file a concise statement setting forth the factual and/or legal basis for the disagreement with MCARE's December 18, 2003, determination. The statement may be in narrative form or in numbered paragraphs, but in either event shall not exceed two pages. A prehearing telephone conference initiated by this office is scheduled for July 15, 2004, at 1:30 p.m. This telephone conference shall also include the two other Nazareth Hospital cases, doc. nos. MM04-05-041 and MM04-05-042, and consider the possible consolidation of the three cases. Each party shall provide a telephone number to be used for the telephone conference to the Hearings Administrator on or before June 18, 2004. A hearing date shall be determined, if necessary, at the prehearing telephone conference.
Motions preliminary to those at hearing, protests, petitions to intervene or notices of intervention, if any, must be filed on or before July 2, 2004, with the Hearings Administrator, Administrative Hearings Office, Capitol Associates Building, Room 200, 901 North Seventh Street, Harrisburg, PA 17102. Answer to petitions to intervene, if any, shall be filed on or before July 9, 2004.
