   Under filing no. 96-L, Capital Blue Cross and Pennsylvania Blue Shield propose to incorporate an optional amendatory exclusion Rider Form #F-898 into the Comprehensive Major Medical Contract for Experience-Rated Groups. The optional exclusion permits Experience-Rated groups to exclude coverage to those corporate officers who choose to ''opt-out'' of the Workers' Compensation program and excludes persons who fail to enroll for such coverage or fail to claim the benefits.

   Copies of the filing are available for public inspection on Monday, Wednesday and Friday, by appointment, during normal working hours at the Insurance Department's Office in Harrisburg. This form filing consists of 3 pages.

   Interested parties are invited to submit written comments, suggestions or objections to Richard W. Stoner, Insurance Department, Bureau of Accident and Health Insurance, 1311 Strawberry Square, Harrisburg, PA 17120, within 30 days after publication of this notice in the Pennsylvania Bulletin.
