Petition of Bentleyville Telephone Company d/b/a FairPoint Communications to seek confirmation that they may use the service price index component of the price cap formula in its Amended Streamlined Form of Regulation and Network Modernization Plan (Chapter 30 Plan) to offset charges in the tax rates as tracked by the State Tax Adjustment Surcharge.
