     By filing No. 1A-HDHP-NQHDHP-SB-12-HBS, High- mark,Inc., d/b/a Highmark Blue Shield, requests approval to adjust rates for its Medically Underwritten Direct Pay PPO High Deductible Health Plans, Medically Underwritten Direct Pay Non-Qualified HDHP and Simply Blue Medically Underwritten Direct Pay PPO. The average requested rate increase is 4.6%. The proposed rate increase would affect approximately 13,100 contract holders and would produce additional revenue of about $2.1 million annually. The requested effective date of the change is October 1, 2012.

     Unless formal administrative action is taken prior to July 26, 2012, the subject filing may be deemed approved by operation of law.
