     On February 7, 2013, Sarah L. McCloskey, LPN, license no. PN275439, of Philadelphia, Philadelphia County, had her nursing license actively suspended effective November 15, 2012, for violating a previous State Board of Nursing (Board) order.

     Individuals may obtain a copy of the final adjudication and order by writing to Judith Pachter Schulder, Board Counsel, State Board of Nursing, P. O. Box 2649, Harrisburg, PA 17105-2649.

     Individuals may file a request for a hearing to challenge the validity of the order compelling the examination or to contest the allegations of the motion to deem facts admitted along with an answer within 20 days of publication of this notice. If a request for hearing and answer are not filed within the time period set forth previously, a final adjudication and order indefinitely suspending the individual's license will be issued by the Board.
