[Saturday, April 23, 2016]Highmark Inc. is requesting approval to increase the premium an aggregate 77% on 477 policyholders with the following individual LTC policy form numbers: SPL-336-HMBSPA and FSPL-336-HMBSPA.
Unless formal administrative action is taken prior to July 6, 2016, the subject filing may be deemed approved by operation of law.
