 On February 3, 2016, LaJuana Johnine Jordan, RN, Pennsylvania license no. RN592087, of Warren, OH, was issued a $500 civil penalty and indefinitely suspended based on disciplinary action being taken against her license by another state and failing to report same to the State Board of Nursing (Board).

 Individuals may obtain a copy of the adjudication by writing to Bridget K. Guilfoyle, Board Counsel, State Board of Nursing, P. O. Box 69523, Harrisburg, PA 17106-9523.

 This order represents the final Board decision in this matter. It may be appealed to the Commonwealth Court of Pennsylvania by the filing of a petition for review with that court in accordance with the Pennsylvania Rules of Appellate Procedure. Individuals who take an appeal to the Commonwealth Court must serve the Board with a copy of their petition for review.
