 On December 17, 2015, the State Board of Medicine (Board) suspended for no less than 3 years, retroactive to October 26, 2011, the license of Sean Patrick Rhoads, PA-C, Pennsylvania license no. MA052146, of Martins- burg, WV, based on his violation of a previous Board order.

 Individuals may obtain a copy of the final order by writing to Teresa Lazo, Board Counsel, State Board of Medicine, P. O. Box 69523, Harrisburg, PA 17106-9523.

 This final order represents the final Board decision in this matter. It may be appealed to the Commonwealth Court of Pennsylvania by the filing of a petition for review with that court in accordance with the Pennsylvania Rules of Appellate Procedure. Individuals who take an appeal to the Commonwealth Court must serve the Board with a copy of their petition for review.
