FD-013751-L and FS-015333, of Philadelphia, Philadelphia County, had his licenses revoked and was ordered to pay civil penalties totaling $5,000, based upon his professional misconduct in the practice of funeral directing.

This final order represents the State Board of Funeral Directors' (Board) final decision in this matter. It may be appealed to the Commonwealth Court of Pennsylvania by the filing of a petition for review with that court in accordance with the Pennsylvania Rules of Appellate Procedure. Individuals who take an appeal to the Commonwealth Court, must serve the Board with a copy of their petition for review.
