Application for Voluntary Surrender of Certificate of Authority [36 Pa.B. 6745]
[Saturday, November 4, 2006]Ion Health, Inc., a domestic health maintenance organization, has submitted an application for approval to surrender its Insurance Department Certificate of Authority. Persons wishing to comment on the grounds of public or private interest concerning the surrender are invited to submit a written statement to the Insurance Department (Department) within 30 days from the date of this issue of the Pennsylvania Bulletin. Each written statement must include the name, address and telephone number of the writer, identification of the application to which the comment is addressed and a concise statement with sufficient detail to inform the Department of the exact basis of the comment and the relevant facts upon which it is based.
