   Blue Cross of Western Pennsylvania and Keystone Health Plan West, Inc., have filed a new Freestanding Ambulatory Surgery Facility Agreement, to be effective January 1, 1997. The Agreement addresses the requirements that will govern freestanding ambulatory surgery facility agreements in the provision of services to members enrolled in Blue Cross of Western Pennsylvania and Keystone Health Plan West, Inc., indemnity and managed care benefit programs.
