PN274666, last known of Philadelphia, Philadelphia County, had her license to practice practical nursing indefinitely suspended for at least 2 years and then until her criminal probation ends, including the payment of restitution, costs and fees, under Doc. Nos. CP-51-CR-1775-2012 and CP-09-CR-2558-2012, she pays a $500 civil penalty and completes 8 hours of ethics continuing education, for denying that she committed any misdemeanors on her application for licensure.

This order represents the final State Board of Nursing (Board) decision in this matter. It may be appealed to the Commonwealth Court of Pennsylvania by the filing of a petition for review with that court in accordance with the Pennsylvania Rules of Appellate Procedure. Individuals who take an appeal to the Commonwealth Court must serve the Board with a copy of their petition for review.
