    Notice to Gary W. Golden, D.C.:

       On June 30, 1997, the Commonwealth of Pennsylvania, State Board of Chiropractic instituted a formal administrative action against you, alleging that you have violated certain provisions of the Pennsylvania Chiropractic Practice Act. Pursuant to 1 Pa. Code § 33.31 and Pa.R.C.P. Rule 430 providing for service of process upon you by publication:

       If you wish to defend against the allegations in the order to show cause, or to present evidence on your behalf in mitigation of any penalties which may be imposed upon you or your license, certification, registration, permit or other authorization to practice your profession, the procedures for doing so are set forth in the order to show cause. You have the right to be represented by an attorney in this matter. Although you may represent yourself, you are advised to seek the advice of anattorney. All proceedings are conducted in accordance with the administrative agency law and the general rules of administrative practice and procedure.

       You are directed to respond by filing an answer in writing within thirty (30) days of the date of publication of this notice. An original and two copies must be submitted. Also, you must send a separate copy of the answer, and any other pleadings or documents, to the prosecuting attorney named in this document. If you do not file an answer, disciplinary action may be taken against you without a hearing.
