   On July 1, 2004, Kathleen M. Bales, Lake Ronkonkoma, NY, was suspended for at least 3 years, retroactive to May 27, 2004, based on her violation of the terms of her Voluntary Recovery Program Consent Agreement and Board Order.

   This adjudication and order represents the final State Board of Nursing (Board) decision in this matter. It may be appealed to the Commonwealth Court of Pennsylvania by the filing of a petition for review with that court in accordance with the Pennsylvania Rules of Appellate Procedure. Individuals who take an appeal to the Commonwealth Court must serve the Board with a copy of the petition for review. The Board contact for receiving service of appeals is the Martha H.
