[Saturday, October 15, 2011]AF&L Insurance Company is requesting approval to increase the premium an aggregate 5.6% on 961 policyholders with the following individual home health LTC policy form: HHC-4.
Unless formal administrative action is taken prior to December 28, 2011, the subject filing may be deemed approved by operation of law.
