[Saturday, October 12, 2013]AF&L Insurance Company is requesting approval to increase the premium an aggregate 55% on 1,447 policyholders with the following individual LTC policy form numbers: LTC-4, LTC-5, LTC-7, LTC-8, LTC-8(TQ), QLTC-97 and LTC-93.
Unless formal administrative action is taken prior to December 25, 2013, the subject filing may be deemed approved by operation of law.

Copies of the filing are also available for public inspection, by appointment, during normal working hours at the Department's Harrisburg Office.
