Application and Request for Plan Approval of a Mutual-To-Stock Conversion [28 Pa.B. 313] Provident Mutual Life Insurance Company, a Pennsylvania domiciled mutual life insurance company, has submitted a Plan of Conversion which provides for conversion from a mutual life insurance company to a stock life insurance company and for the formation of a stock and a mutual holding company. The filing was made under the Mutual-to-Stock Conversion Act (40 P. S. § 911-A et seq.).
