DEPARTMENT OF HEALTH Health Maintenance Organization Certificate of Authority [27 Pa.B. 216] On November 27, 1996, Pennsylvania Physician Healthcare Plan filed with the Departments of Health and Insurance, an application for a Certificate of Authority to establish, operate and maintain a health maintenance organization in accordance with the provisions of the Health Maintenance Organization Act (40 P. S. §§ 1551--1567), Department of Health Regulations (28 Pa. Code §§ 9.1--9.97) and the Insurance Department HMO Regulations (31 Pa. Code § 301.204).
The proposed service area of the applicant is Adams, Berks, Carbon, Cumberland, Dauphin, Franklin, Lackawanna, Lancaster, Lebanon, Lehigh, Luzerne, Monroe, Northampton, Schuylkill, Wyoming and York counties.
